Thank you for your interest in becoming part of the family here at Pediatric & Adolescent Center of NW Houston.  Prepare for your appointment by following these instructions.


BEFORE YOUR APPOINTMENT


Insurance Information
Please ensure we have your insurance information prior to your appointment date. See our Billing & Insurance Page for a list of accepted insurances. Our staff will make every attempt to verify your benefits prior to your appointment. Newborns: Please be advised, most insurances ask that you enroll your newborn within thirty days of his or her date of birth.

Medical Records
Newborns: When you and your new baby are discharged from the hospital, you will be provided with discharge paperwork and possibly a shot record listing your baby's first Hepatitis B vaccine. Please bring these documents with you to your first appointment. Transfer Patients: If your child is transferring to our practice, you may want us to have a copy of their records. To transfer your child's records, please complete our Medical Release Form and provide it to your child's previous physician. This form will give your previous physician permission to either provide your child's records to you or fax them directly to us. If you choose to have your child's records provided to you, please bring them to your first appointment for us to copy.

New Patient Paperwork
Please print and complete our New Patient Paperwork to expedite your check-in process on the day of your first appointment. If you are unable to print and complete the New Patient Paperwork, please arrive to your appointment twenty minutes early.

What to Bring

  • Copayment. Please see our Billing & Insurance Page for detailed information on payment expected at the time of service.
  • Insurance Card
  • Driver's License
  • Medical Records (if they were not faxed directly to us)
  • Immunization Record
  • New Patient Paperwork (if you completed it ahead of time)

Arrival Time
Please arrive for your child's appointment ten minutes early if your paperwork is completed ahead of time, or twenty minutes early if you would like to complete your paperwork in the office. We are closed for lunch from 1:00 - 2:00; therefore, 2:00 appointments are not required to arrive early.


AFTER YOUR APPOINTMENT


Prescriptions
If one of our physicians prescribed medication, it will be electronically submitted to the pharmacy of your choice. Please allow approximately 1 hour for your pharmacy to prepare your child's prescription.

Insurance Claims
If you have provided insurance information, we will file your claim to your insurance company. If there is any additional patient share required, you will receive a statement in approximately eight weeks.

Lab Results
Please allow 7-10 business days for routine results. Urgent lab results will be provided to you by one of our physicians on the same or next day
